If my teen son wants to return to the ship alone when he is ready, may he, or must a parent be with him to get him back on?

You will just need to fill that out when you do your online check in! They ask those questions on the forms. We allowed our 14 year old to do this and although I did worry (ugh!) it was perfectly fine! He did great!
He can return on his own. My 14yo DD returned to the ship on her own from CC when we were there a few weeks ago. No issues.
There's no form for this on DCL. My DD did it and I never filled out a form for her. The forms are for if you want your child to be able to get off the ship with an adult from another stateroom. A minor can't get off the ship without an adult but they can return to the ship with no issues.
Actually there is a new spot in online check in that has you answer if the minor can leave and return to the ship unaccompanied from port.

That still only pertains to leaving the ship with or without an adult. A child will always be allowed to return unaccompanied.

Yes, but obviously if they can leave the ship unaccompanied, then they can return unaccompanied. It's not like they can just go pick up a random adult to escort them back. OP was asking if they child can come back unaccompanied. Another post said that there is no form that asks that.
